Social Worker
At this time, we are seeking a Temporary Social Worker to join our team!
This is a temporary position (4-8 weeks).
As part of the admission process, meet with the resident and/or family to review policies on advanced directives, resident rights and discrimination.
Conduct and document a Psychosocial History and Assessment, including identification of resident problems/needs.
Attends interdisciplinary team meetings.
Coordination of discharge planning and family meetings.
When appropriate, provision of psychosocial groups.
Provision of one-to-one interventions
Provide for and or arrange for social work services as indicated by Resident needs.
Assist resident with concrete needs, such as financial planning.
Communicate with resident's family re:
resident needs, transfers, discharge and changes in level of care, and financial responsibility.
Implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and the physical environment to enhance quality of life.
Interpret and explain resident behavior to staff to foster understanding, facilitate treatment and foster respect for resident rights.
Respects confidentiality.
May serve as back-up to admission coordinator
All other related duties as assigned.
